Account Migration Procedure
Intended Audience
These instructions are intended for current users of FormAssembly.com with an active account created before October 1st 2007.
My account doesn't work on the new site, what should I do?
That's normal. Please refer to the migration procedure below. Your account is not affected by the change unless you actually want to migrate to the new site.
Why should I migrate my account?
The new version of FormAssembly.com is a major update to our service. It offers new features and a new interface. From a technical perspective, it is a completely new application, running on a new platform.
Migrating your account will give you access to the new features and ensure that you can get the most out of our service.
When should I migrate my account?
In order to facilitate the transition, we will be running the two versions concurently for as long as necessary. There is not cut-off date, so there is no risk of disruption of service. We do encourage you nonetheless to migrate your account as soon as possible.
What about the forms on my website, will they use the new version automatically?
No, they will continue to send responses to your old account, which remains fully operational. To switch your forms to the new version, you will need to replace the form on your site with the one obtained from your new account.
Procedure
- Sign-up for an account on the new version here:

This will create a new account. You can enter the same username and password you were using, but there is no guarantee that it will be available. If your username is not available or invalid, please use a different one. - In the plan selection page, select the 'Free Plan'. You will be importing your account status in the next step so there is no need to pay for your plan again here.
- Once your account has been created, log into the new FormAssembly.com at:
http://app.formassembly.com - Next, click on the 'My Account' tab, then 'Plan' and 'Migrate Account'.
- On the account migration page, enter your old FormAssembly.com username and password and click 'Check'.
- If the username and password are valid, you will be presented with your user information and the list of your forms.
- Check the 'Transfer Plan' and 'Contact Information' checkboxes, and check any form that you want to import. Then click 'Import'.
- Your forms have now moved. You may need to update your website:
- If you are hosting a form on your website, you must re-publish it. (See publishing instructions.)
- If your forms are hosted on FormAssembly.com, you need to update all the links that point to your forms. (The new address is visible under 'My Forms' > 'Display&Processing'.)
- Contact us if you need more assistance with this.
- You're done! You can start using the new FormAssembly.com.
