    I have used the HTML publishing option to add my form to my website, using some custom CSS and stuff to make things match. Is there any way to make it so that when people want to resume the form, the resulting login and form pages are shown using the same template page I've made at my web site?

    Yes, using our API. It's really easy to setup, but you need to be able to copy&paste some server-side code (PHP for instance) in your page.
