    I have the 5c per message plan... how do i create an upload field for my form??? I can't find it anywhere!

    Hi,

    First welcome to the FormAssembly.com support forum.

    The upload field is available in the Form Builder, so first open your form in the Form Builder (or start a new form).
    Then click on 'add a question', and in the 'properties' tab, select 'file upload' as the 'question type' (that's the first option in the third row).
