Hi - I have 3 telephone fields in my form - Home Phone, Cell Phone and Other Phone. I used the predefined fields. But I cannot seem to get them line up on one line - they all are on their own line. I put all 3 fields in an Inline Section.
Actually, from a usability point of view, it's much better if you have all your fields vertically aligned on the screen. This approach makes filling out your form easier, as the user can just move regularly down the page without having to sometimes jump off to the side.
Research and experience shows that if you have one or two fields horizontally aligned when the majority of the form is vertically aligned, there's a very good chance that people will miss those extra questions hanging off to the side.
Also, doing it all vertically means you don't have to worry about different screen and window sizes leading to funny wrapping and alignment of your fields. This is important given than screen resolutions typically vary from 800 pixels wide to 1280 pixels wide. Try resizing your browser window when you've got the form open and you'll see what I mean.
So in your case, I would recommend having your questions running down the page, one after the other, as follows:
Full name Name on badge Street address City Zip Code State Home phone Cell phone Other phone Email address
I would also align your radio buttons for the question about position in group vertically.
I hope this helps.
