I can't seem to figure out how to create the form I need.

It's a t-shirt order form. I would like to have a blank for name and phone number at the top.

Then I need the total price to calculate when the quantity, color, and size change. For example; a white shirt size s-xl is 7.50, anything bigger than xl add 1.25 for every x. Any colored shirt, is 9.50 s-xl, then a 1.25 for every x after that. So I need an order form that would calculate and leave a total at the bottom.
